So I made a new configuration named 'Test', all my USB keyboards showed up, nothing was grayed out I re-opened MIDI Monitor and they were all listed there as well.

You should see your Controller listed by name, such as 'Impulse.' If you only see 'USB MIDI Device' or 'USB Audio Device' you may have this issue.

If it IS listed in the USB Device Tree next open Applications > Utilities > AudioMIDISetup > Window > Show MIDI Studio.

Open the Apple Menu > About this Mac > More Info > System Report >USB and check that the Controller appears here in the USB Device Tree.

Make sure the Controller is connected directly to the USB port on the Mac.
